What is Premature Ejaculation?
At EdenMed Clinics, in Chelsea and Bournemouth, we like to speak openly about wellness issues that may affect both men and women. Until recently, men have had a notorious reputation for being less open than our female partners in matters relating to sex and sexual performance. But we all want to feel great and make our partners feel great in the process. Did you know, around 1 in 3 men experience PE? It's more common than we think.
Premature ejaculation (PE) is a sexual condition where a man consistently ejaculates sooner than he would like. It is the most common ejaculation problem. Most men will experience PE from time to time, but it becomes a sexual disorder if it happens repeatedly over a prolonged period of time. Although PE is often associated with penetrative vaginal sex, it can happen during any kind of sexual stimulation, including foreplay, oral sex, anal sex, or masturbation. Although it can be embarrassing or frustrating, this condition is a common problem, which can be successfully treated.
One study timed hundreds of men with a stopwatch to determine the average length of ejaculation time during intercourse (also known as intravaginal ejaculation latency time, or IELT). In the study, the average time of intercourse before ejaculating was five minutes. The majority of men who experienced premature ejaculation lasted less than a minute.
How is Premature Ejaculation diagnosed?
There isn't a firm definition, but accepted criteria include, persistent or frequent ejaculation before penetrative sex, or within 2 minutes of penetrative sex, an impactful psychological component, such as embarrassment or shame (these may lead to further issues, such as erectile dysfunction), and a relational component, such as avoiding sex, communication issues or relationship frustration. Most men will experience early ejaculation at some point throughout their life.
What are the causes of premature ejaculation?
Premature ejaculation can be caused by a variety of psychological and physical issues in isolation or varying combinations throughout time.
Psychological causes of premature ejaculation can be treated with therapy and communication. It is important to communicate openly with your partner and voice any mental, emotional and psychological concerns when speaking to a doctor.
There are a number of physical factors that may predispose one to premature ejaculation. Penile sensitivity, testicular sensitivity, cremasteric hypersensitivity, prostatic issues, as well as other factors, such as prescribed and non-prescribed drugs, may have varying roles. There are a number of medications, topical and oral, that may help if you suffer with premature and ejaculation. These include topical lidocaine anaesthetic cream and oral dapoxetine tablets.
How can EdenMed Clinic help?
As well as being able to prescribe the medications mentioned above, Dr. Sam will take a detailed history and advise on additional therapies if appropriate. An aspect that is often overlooked is both the aesthetics of the penis and scrotum as well as a hyperactive and sensitive cremasteric reflex.
The cremasteric muscle is a paired muscle within the wall of the scrotum. It is thought to be involved in testicular temperature regulation and in the process of protecting the testicles during sex. The reflex is mediated through nerve fibres arising from the L1 and L2 spinal nerve nuclei. While the ejaculatory autonomic system is mainly mediated through the sacral spinal cord, there is significant overlap with thoracolumbar nerves to coordinate pelvic muscle contraction and cerebral activity during sexual climax. Our understanding of this complex interaction and the even more complex wiring system is only now evolving. By way of embryology, the scrotum and cremasteric muscle are part of the pelvic structures, and therefore, involved in climactic ejaculation.
Botulinum toxin has been used in the treatment of testicular pain resulting from inappropriate contraction of the cremaster. At EdenMed Clinic, Dr. Sam has developed a protocol to treat both the appearance and function of the scrotum using botulinum toxin to relax the muscle and allow the testicles to hang lower within the scrotum. The effect of this in treating premature ejaculation is to reduce the pressure sensation felt by the testicles, which in some men, may contribute to climax.
Two important notes. Firstly, the use of botulinum toxin for this indication is considered 'off license'. It's use is therefore, at the discretion of both you as the patient and Dr. Sam. Secondly, the absence of a cremasteric reflex, which botulinum toxin (Botox, Azzalure, Alluzience, and others) will produce, may make diagnosing testicular torsion, if it occurs within the time frame the the treatment persists, more difficult. However, the cremasteric reflex is thought to be a contributing factor to developing torsion, so reducing it may actually be protective.
